Public Records Request
This form is designed to help government agencies, the media and the private citizens in Polk County access public records related to Polk County Public Schools. The District understands that it must be accountable to its citizens and their right to know how their tax dollars are spent and how decisions are made.

Costs for fulfilling public records requests are as follows:
- $.15 – single-sided copy
- $.20 – two-sided copy
- $5 – DVD/CD
The actual cost of duplication will be collected for copies of district public records in a form other than simple paper copies.
The actual labor cost of the personnel providing the service will be collected as permitted by state law if the request requires more than 15 minutes of clerical or supervisory assistance, including redaction services, by school district personnel.
A special service charge attributable to extensive use of information technology resources and/or labor cost of the personnel completing the request may be collected as permitted by state law.
The charge will be reasonable and based on the cost incurred for such extensive use of information technology resources and/or the actual labor cost of the personnel providing the service, after the first fifteen (15) minutes. (Example: if the hourly wage is $15.00 and ninety (90) minutes is required to gather materials, the fee shall be $15.00 x 1.25 = $22.50, plus the allocated per copy fee.) The current hourly wage is $25 for District personnel or $35 for IT resources, including email searches.
