New Hire Insurance Benefits Information

Newly hired employees become eligible for insurance benefits the first of the month in which they reach 90 days of employment.

  • The New Hire Enrollment Form can be returned completed by courier to the Employee Benefits Department, Route E.

If you choose to waive health coverage, and you do not want to elect any supplemental plan options offered, please mark the “Waive PCSB Health Plan” box and complete the Group Term Life Beneficiary section.

Polk County Public Schools is pleased to offer a comprehensive benefits package including board-funded employee health insurance and $20,000 Group Term Life including $10,000 AD&D coverage for the dedicated, hard-working employees that make daily contributions in the lives of students and fellow employees. The Risk Management and Employee Benefits Department continue to focus on Ongoing Well Living (OWL) for all employees and dependents! Our goal is to create WISE employee benefit consumers because Wise Individuals Stay Engaged!
The Risk Management and Employee Benefits Department are pleased to announce the PCPS Employee Health Clinics are available for all PCPS Employees to use from their date of hire regardless of health plan coverage and available to dependents (over the age of 2) once covered on the PCPS Health Plan.

Transparency in Coverage

On 11/12/2020, the Department of Health and Human Services (HHS), the Department of Labor, and the Department of the Treasury published the Transparency in Coverage Final Rule (85 FR 72158-01). The first phase of the Transparency in Coverage Rule requires group health plans and health insurance issuers to provide pricing information for covered items and services as follows:

  • In-network provider negotiated rates
  • Historical out-of-network provider allowed amounts

Benefit Effective Dates

PCPS New Hire Employee Benefits are effective the 1st of the month in which the employee reaches 90 days of employment. Please note employee-only health and the basic term life coverage are automatically effective. Enrollment for any additional coverage is considered to be a voluntary benefit and you must submit a New Hire Enrollment Form.

Employees who work at least 30 hours per week become eligible for insurance benefits the first date of the month in which they reach 90 days of employment. Benefits elected by the employee will be effective on this date.

Polk County Public Schools is pleased to offer group health, dental, vision, life and disability plans to benefits-eligible employees.

Yes, the Board funds employee health insurance and $20,000 basic term life insurance coverage. These benefits automatically become effective on your eligibility date.

Employees have 30 days from the date of hire to submit a completed New Hire Enrollment Form to Risk Management.

If an employee does not submit an enrollment form within 30 days they will be only be enrolled in the PCPS health plan with employee-only health coverage and $20,000 basic term life coverage. If an employee wishes to elect coverage outside of the 30 day enrollment period they must experience an IRS qualifying event or elect coverage during Open Enrollment.

If an employee does not wish to carry the PCPS health plan they MUST submit a New Hire Enrollment Form electing to “Waive the PCPS Health Plan”. If an employee does not waive coverage during this initial period they must experience an IRS qualifying event or wait until Open Enrollment.

Employees may make changes to their PCPS insurance benefits during the middle of a plan year if they have experienced an IRS qualifying event.
